NEW PATIENT MEMBERSHIP PRICING
$2500 for one year of care*
+$275 per female pellet insertion, 4 per calendar year (pricing may differ for patients on an increased pellet schedule)
+$450 per male pellet insertion, 2 per calendar year (pricing may differ for patients on an increased pellet schedule)
What that includes:
-Initial consultation and office visits throughout the year
-Initial blood panel, plus 2 subsequent hormone blood panels throughout the year
-Text, email, phone call accessibility for quick specific questions.
-Refills, receipts for insurance, records requests, etc.
*$1,000 deposit due upon booking, the remaining $1500 is due at the time of your consult, should you decide to move forward with your individualized treatment plan.

ESTABLISHED PATIENT MEMBERSHIP PRICING
$885 for the year of care
+$250 per female pellet insertion, 4 per calendar year (pricing may differ for patients on an increased pellet schedule)
+$450 per male pellet insertion, 2 per calendar year (pricing may differ for patients on an increased pellet schedule)
What that includes:
-2 hours of visits
1 hour for yearly consultation, 1 hour of flexible visits (you can split this up however you'd like whether that be another 1 hour visit, or two 30 minute visits)
-Text, email, phone call accessibility for quick specific questions.
-Refills, receipts for insurance, records requests, etc.
Established patients requiring more than 2 hours of care a year will be given a special prorated hourly rate.
